“The manual was published, so the culture changed” replaces actual use with availability. Recover the practice observations. “Everyone completed training” becomes a reliability claim. Identify the training result and the still-unestablished maintenance effect.

A cultural problem is answered by adding awareness training when access or incentives prevent enactment. Change the relevant arrangement or preserve that limit in the recommendation.
