A.15.11:8 - Common Anti-Patterns and How to Avoid Them

Publishing without arranging an encounter. An unused method receives a clearer title and another index entry, but the intended practitioner still never reaches either. Improve the occasion and route into the work.

Matching a topic while missing a condition. A cue for comparing optional additions is shown when the task is to fulfil a binding reporting obligation. Retain the obligation and compare permissible ways to meet it.

Repeating a cue after recognition succeeds. A learner or agent can identify the need but lacks a constituent capability. Obtain the missing contribution; repetition does not resolve that failure.

Counting uptake as value. Counting more opened patterns or applications can conceal delay or displaced work. Examine what improved, including cases where no intervention was preferable.
