Closing the form while the condition is absent. An account-creation ticket says “done”, but the person cannot read the authorized material. Return the usable-access question to the provider with the actual failed use and applicable grant.
Treating one label as one subject. A report counts “completed trips” without distinguishing journeys, business results and administrative cases. State which result is counted and retain any correspondence needed for the decision.
Expanding every request into service redesign. A handler sends an eligible ordinary request into a discovery workshop. Use the adequate existing instruction; reserve redesign for a repeated or unresolved arrangement problem.
