Documents, training attendance and organizational labels are visible, while actual use is harder to observe. A team can receive a new Method description yet continue its previous work. A practice can spread because it is convenient or rewarded without improving maintenance outcomes.
Deliberate intervention and cultural effect also differ. Choosing to alter training or recognition does not establish that practitioners changed their work, that the change persisted or that equipment functioning improved.
