The local care plan requires a qualified clinical decision and confirmation of necessary home support before discharge. The selected case rule allows the responsible practitioner to arrange assessment and support in the order the situation permits, while keeping both conditions visible. A change in support can reopen planning. The standard medication-check subprocedure remains applicable when the case reaches it.
