ADM.13:2 - Problem

A current status overwrites the earlier decision and its grounds. Later staff cannot explain why an action was taken, which terms applied or whether a payment was actually made. Alternatively, an organization keeps every document indefinitely while losing the interpretation or access route needed to use it.

Correction, restricted use and disposal are often treated as one operation. Someone deletes an old account to fix a current display, or keeps exposing information because another legitimate purpose requires retaining it. Both errors substitute a storage action for the actual institutional question.
