HCD.3:1 - Problem Frame

When Work goes badly, the visible human is often the easiest place to intervene. Yet the same observation can arise because the demand was wrong, the Method or source was missing, a tool or cue changed, the assignment or authority failed, workload or recovery was inadequate, a specialist return was absent, the response was not recognized as applicable, or the person’s capability really is limiting.

These branches have different owners and remedies. More practice can be useless when the task lies outside the claimed envelope, unsafe when a clinical or safety condition is current, and misleading when assistance or prompt design controls the result. Conversely, an endless list of possible causes is not a diagnosis. The comparison must become small enough to change a decision.
