HCD.9:1 - Problem Frame

Practice is often recorded as exposure: pages completed, hours logged, cases submitted, or sessions attended. That record does not say what the person attempted, which help was present, what error was observed, whether the feedback reached the person in time, or what changed in the next attempt.

The opposite failure is answer substitution. A teacher demonstrates the correct result, a peer rewrites the response, or AI generates a fluent correction; the final work product improves, but the learner’s contribution becomes unknown. When the first attempt is overwritten, later assessment cannot distinguish independent action, supported action, and copied repair.

A useful practice loop therefore preserves the initial conditions and response, supplies information that the learner can act on, and checks the affected action again. It keeps local correction valuable without promoting it to a capability, transfer, or retention conclusion.
