HCD.9:8 - Common Anti-Patterns and How to Avoid Them

“Practise for ten more hours”Duration does not identify the action, error, feedback, or observation.Select one practice focus and close one traceable loop.
“Feedback was provided”Content, recipient, timing, and opportunity to use it are unknown.Record the exact criterion-bearing contribution and next action.
“Here is the correct answer”Provider output can replace the learner action.Use the smallest adequate cue and obtain a learner-authored correction.
“The corrected file proves mastery”The first attempt and support have disappeared.Preserve attempt, help, correction, and retry as different facts.
“Repeat the same question”Answer memory can mimic the intended discrimination.Change the condition that should change the warranted move.
“Retake the whole module”Already supported requirements and the exact gap are lost.Retry the affected requirement and preserve compatible success.
“They improved after feedback”Temporal order does not establish causality or durable capability.Report changed performance under the recorded conditions and seek stronger evidence separately.
“Everyone passed overall”A critical or individual unresolved gap is hidden by aggregation.Keep holder-level requirements, safe stops, and continuations visible.
