The practice question is how to respond when an apparent OCE practice fails to continue. The selected line is case-based recognition followed by a mechanism-matched, condition-qualified response and later observation. The serious alternative is a common communication or training package applied to every apparent non-use. It is cheaper to schedule, but cannot distinguish a deficient example from missing permission or a valid refusal of an obsolete move.
Recover the practice being continued. Identify operative Methods across descriptions, instruments and practitioners, and examine repeated Work in unlike settings. Sections 4.1–4.2 and the two cases show how exposure, support, use and continuation can differ.
Adapt current implementation inquiry. The NPT coding manual (May et al., 2022) and NPT-derived strategy synthesis (May et al., 2025) are best-known-line contributors for matching implementation work to an identified difficulty. They change sections 4.3–4.5: ask about understanding, participation, support and appraisal before selecting a response. The synthesis used 63 health/social-care studies and interpretive coding; it supplies no comparative effectiveness result for OCE practitioners.
Retain the contextual challenge. The NPT consolidation, version 1 (May, Finch and Rapley, 2026) strengthens attention to changing conditions and distributed burdens in sections 4.2 and 4.5. It does not establish cultural-evolution or individual-learning mechanisms.
At comparable bounded effort, inspecting one useful and one doubtful case can distinguish next actions that the generic package leaves conflated. The deliberate cost is obtaining case evidence and professional input where the question needs them. Reject the claim that this comparison establishes intervention efficacy. Reopen the selected response when later cases, a better applicable rival or changed source evidence defeats its mechanism, protection or continuation premise.
