OPS.20:4.3 - Separate the practice, carriers, Work, claims, and consequences

Keep each actual subject and claim visible:

PositionWhat it can establishWhat it does not establish by itself
admitted Method or candidate accountThe exact reusable way or the exact pre-admission question and statusPublication, enactment, or a population relation
MethodDescription, card, board field, repository, checklist, or archiveRecoverable content, access, current wording, or retained carrierThe Method, correct use, selection, or retention
publication or training WorkA dated publication or learning occurrence with actual participantsReceiving enactment, population adoption, or later retention
operating WorkWhat actual practitioners did, with admitted enacted Methods where supportedA population-wide relation or causal explanation
local decision or ruleWhat an authorized chooser selected for its scopeActual enactment, cultural selection elsewhere, or effectiveness
cultural claim epistemeA separately stated claim about the named value, participants, predicate, and intervalThe world-side relation it describes
service, reliability, flow, toil, adoption, or human-condition observationA direct consequence for its own subject and intervalThe cultural predicate or proof that an intervention caused it

For the primary predicate, record the world-side claim, the separate episteme that states it, evidence and reliance, and observations at named intervals. Preserve unknown and missing information. A later observation neither creates the earlier occurrence nor proves that the intervention caused the change.
