HCD.9:4.7 - Return the Practice-Episode Result
An HCD.9 result contains:

| first attempt | Learner action and work product, time, uncertainty, and support state before feedback |
| actual help | Sources, tools, AI, peers, teachers, specialists, cues, demonstrations, and when each was received |
| observation | Criterion-bearing success, error, uncertainty, plausible cause, and what remains supported |
| feedback | Exact content, provider, timing, channel, intended next action, risk boundary, and opportunity to use it |
| correction and retry | Learner’s correction, changed or repeated task condition, affected requirements, result, and support |
| disposition | changed action under recorded conditions, unchanged difficulty, new difficulty, or safe stop/incomplete |
| continuation | Next practice focus, needed task or support revision, or bounded stop; keep a decision-relevant unresolved gap and reopen condition. Include a stronger evidence question only for a requested stronger claim. |
Preserve the trace at the resolution needed by the next decision. Do not require one record per pattern when one complete task tests several connected Methods, and do not merge distinct first, helped, corrected, transfer, or delayed attempts into one score.
